Service Agreement Form
An applicant for electric service must sign and submit a confidential Service Agreement Form.
You may either:
1) Bring a photo ID and sign an agreement in person at the office,
2) OR:
a) Download and print the two-page form (available by clicking below)
b) Complete the required information
c) Have your signature(s) notarized, and
d) Return the form to the REMC.
Please note that Notary Publics can usually be found at your county courthouse and other local government offices, local banks, insurance agencies and law firms.
